This is the wreckage of and Army Air Service Curtiss JN-4D Jenny, Air Service serial no. A.S. 3131, at Gerstner Field (11 miles southeast of Lake Charles, Louisiana). This aircraft is believed to be the first U.S. aircraft converted for use as an air ambulance. Maj. Nelson E. Driver (Medical Corps) and (then) Capt. William C. Ocker (Air Service) were responsible for the conversion. The aircraft's rear cockpit was modified for the transport of a patient in a semi-reclining position. The aircraft was also used to transport doctors. See also: en.wikipedia.org/wiki/William_C._Ocker
