I find indications that this used to be at 412 S Main St Canyonville, OR 97417. E.g., www.oregon.gov/olcc/docs/final_orders/final_orders_1984/f... But by 2012 ( see Streetview from 2012 ), the location was occupied by Marla Kay's Cafe, which seems also to have closed.
