Assigned 479th TFW. To AF Systems Command (ARPS). W/O 1 March 1969. Crashed near Edwards AFB after apparent engine failure/engine fire. Pilot ejected safely. F-104C-10-LO, s/n 57-0922. Assigned 435th TFS, 479th TFW. At Udorn AB, Thailand (8th TFW). W/O 14 May 1967. Crashed while aerial refueling. Fuel shut off valves failed and ruptured the fuel tanks. Pilot ejected safely.
