Looks like they're measuring tension in pounds. They're pulling on the rod coming out of the back of the gauge and getting a reading of around 15 lbs. One might use this tool to check if a spring was defective or if a mechanical assembly required the correct amount of force to move. For example, your car's parking break might require 5 lbs. to set. If it required 20 lbs., you'd need to see if something was broken.
