    On 12 DEC 1966, while on approach to USS Valley Forge (LPH-8), this aircraft crashed into the sea, seemingly from some sort of catastrophic malfunction. All 10 souls aboard were lost. This was the first UH-46A accepted by the USN, and the 26th H-46 built. She was returned from HC-1 Det Atsugi to O & R NORIS for the first PAR (paint & return). NAVAIR decided to add the fuel drop tanks to the external fuselage, where there were no hard points previously, and with no known vibration/stress flight program. At the time, there was also a known possibility of a rotor blade problem in the fleet; a probability of a number of blades with a manufacturing defect in the spar. Since no one actually observed this bird contacting the water (it was still several miles from the ship), the failure is still speculative. Most of a PHIBRON Staff were lost, and no knowledge was gained.
